Your original invoice is your proof of purchase for the Road Hazard Program. The Road Hazard Stamps attached or the Road Hazard Registration Numbers printed onto your invoice are required when submitting a claim under the Program. By registering your Road Hazard, we can still help if you don't know your Road Hazard Stamp or Registration Numbers. The more information you provide us when you register, the better we'll be able to assist you in the event that you misplace or lose your original invoice showing your purchase of the Road Hazard Program.
Using the information on your original invoice, please enter as much of the information requested as possible. If you have any problem registering through this site, you can also register by faxing a clear, legible copy of your original invoice to 1-866-507-9516, or by mailing it to Register My Road Hazard, PO Box 16846, Golden CO 80402.
